ABSTRACT

Objective To investigate the clinical effectiveness of ileum circulation loop in the treatment of familial adenomatous polyposis ( FAP) and ulcerative colitis ( UC) by total proctocolectomy or subtotal colectomy. Methods Totally 31 FAP or UC cases who receiving total or subtotal proctocolectomy in our department from January 1987 to June 2007 were divided into 2 groups according to using the ileum circulation loop or not in the different operation methods. The postoperative complications and the times of defecation in 24-hour and during night after 1 month and 1 year of operation were observed and analyzed. Results In the 31 cases,the incidence of postoperative complications were obviously decreased in using the loop group than in those of the without ( P

ABSTRACT

Blood samples were collected from 10 cases with Billroth Ⅰ gastrectomy(BI),15 cases with Billroth Ⅱ gstrectomy(BⅡ),and 15 cases with pyloroantral flap preserving gastrectomy(PAFPG)were examined to evaluate the changes of plasma insulin and serum gastrin.The blood samples of 8 normal subjects were examined likewise to serve as control.The blood samples were collected in intervals before and after the subject had taken hypertonic glucose solution.It was found that plasma insulin was significantly higher in BⅠ and BⅡ group than in PAFPG group,but it showed no significant difference between the PAFPG group and the control.Serum gastrin was signficantly higher in PAFPG group,BⅠ group,and the control than in BⅡ group.These findings suggest that the changes of insulin and gastrin secretion vary with the type of subtotal gastrectomy.

ABSTRACT

The level of lipid peroxides (LPO)and copper-zinc containing superoxide dismutase(SOD-I) in the gastric mucosa and serum was determined in 141 samples from 25 patients after conventional subtotal gastrectomy (11 cases of Billroth I and 14 cases of Billroth Ⅱ and 11 patients after pylorus and antroseromuscular flap preserving gastrectomy (PAFPG).Those of 11 normal subjects were examined likewise to serve as control.It was found that;1.The average LPO level was much higher and the average SOD-I level much lower in the gastric mucosa after conventional subtotal gastrectomy especially the Billroth type Ⅱ than in those after PAFPG.2.In 36 specimens of stump mucosa,the average LPO level was significantly higher in the tissue around the anastomotic ring than in that of the body of the stump;no marked difference of SOD-I level between the 2 was revealed.3.The LPO value in the stump mucosa was positively correlated to and the SOD-I value negatively correlated to the pH value of gastric juice.Thess findings suggest that the reaction of oxygen free radicals in the stump-mucosa may be influenced by the intragastric pH or by the type of digestive continuity reconstruction,and that the reaction of free radicals specially lipidperoxidation may play a role in the pathogenesis of the lesions in the anastomotic stoma.

ABSTRACT

In order to evaluate the effects of subtotal gastrectomy on the release of vasoactive intestinal peptide (VIP) into the blood,blood samples were taken from 10 patients after Billroth Ⅰ operation (B-Ⅰ),15 patients after Billroth Ⅱ operation (B-Ⅱ),15 patients after antroseromuscular flap preserving gastrectomy (PAFPG),and 8 normal subjects who served as the control.The blood samples were taken after the individual took hypertonic glucose solution.It was found that the VIP elevation rate was significantly higher in B-Ⅰ and B-Ⅱ group than in PAFPG group (P 0.05).In B-Ⅰ and B-Ⅱ,the VIP elevation rate was significantly higher in those cases with dumping syndrome than in those without (P
